ALLERGY WAIVER

I acknowledge that if my child has any allergies, they are listed on this waiver or provided prior to this date. I understand that it is my responsibility to inform The STEAM Collective of any allergies or medical conditions that may require attention.

I release and hold harmless The STEAM Collective, its employees, and affiliates from any liability arising from allergic reactions or incidents that may occur due to exposure to allergens.

SOCIAL MEDIA & PHOTO RELEASE WAIVER

This Social Media & Photo Release Form acknowledges and agrees to the terms below:

1. The parent of caregiver grants permission to The STEAM Collective to use their child’s photographs on Company social media accounts including Facebook, Instagram, TikTok, and Twitter, and on the Company website, without acknowledgment or recognition given to the Party.

2. The Party grants The STEAM Collective creative permission to alter the photographs, provided that the photographs are not altered in an explicit manner or cause malicious representation of the Party and their associates.

3. The Party consents to any third party (including any agency, client, publication, or other organization or institution) for distribution (“sharing”) of the photographs on their social media accounts, for the purposes of publicity and promotion of The STEAM Collective.
